Making an Appointment
  Routine appointments may be scheduled by calling the office during regular office hours.
When you schedule your appointment, our staff will inquire about the reason for your visit,
the name of your referring doctor, and your insurance information.
 

   Many insurance carriers will not pay for your visit unless we have received
a written referral from your primary care physician.

 

  Patients are seen by appointment only.  Same day appointments and emergencies are
always accommodated.  Kindly notify us if you are unable to keep your scheduled appointment,
so that the time might be made available to someone else

 

  Please bring all of your medications (or a list of medications with dosages)
and any copies you may have of your recent blood tests, x-rays or endoscopy reports. 
Please bring your drivers license or government issued ID card and your insurance cards. 

 

  If you are scheduling an appointment for a procedure, please be sure to obtain and
follow all written instructions.
